Tallgrass Surgical Center: Insurance and Billing Information

Tallgrass Surgical Center accepts most insurance plans. To find out if we are a participating provider in your plan, please feel free to contact our Business Office at (785) 272-8807.

Our facility fee covers your preoperative evaluation, most supplies and medications, equipment, personnel and the use of the operating and recovery room. Your overall facility fee will be higher if multiple surgical procedures are performed or an overnight stay is required after surgery. In addition to the facility charge, the following services will be billed separately:

• *Surgeon
• *Anesthesia, if you receive anesthesia or it becomes necessary for an anesthesiologist to be
  available for your surgery.
• Laboratory tests, if performed preoperatively or postoperatively.
• Pathology, if tissue or specimens are removed during surgery.
• *Radiology exams, if performed preoperatively or postoperatively.

*Please be advised although there are several entities with “Tallgrass” in their business name, a separate bill and/or Explanation of Benefits will be generated from each. (Example: Tallgrass Surgical Center, Tallgrass Orthopedics and Sports Medicine, Tallgrass Anesthesiology Associates, Tallgrass Oral and Maxillofacial Surgery and Tallgrass General, Vascular and Bariatric Surgery.)

Our Business Office will call your insurance carrier prior to surgery and verify your surgical benefits for our facility fee. Generally, if your insurance requires special authorizations, referrals or claim forms, they are completed prior to your arrival at the Center. However, if you have information needed for submission of your Tallgrass Surgical Center claim, please bring them with you on the day of your procedure along with your insurance card(s).

The timeliness of payment varies according to payer. You should receive a statement of account after we have received payment or some type of communication from your insurance company regarding your surgery. If you have not received a statement from us within 30 days, please do not hesitate to contact us regarding your account status.

Payment
To assist you, we provide the following options:

1. Cash – includes money orders and personal checks. We request personal checks arrive at our Center two weeks prior to the date of surgery, if possible.
2. Credit cards – we accept Visa, MasterCard and Discover.
3. Care Credit® - this plan allows you to pay over time with convenient low minimum monthly payments. For more information, please contact our office or visit their website at www.carecredit.com

Payment Plans
We request balances are paid according to the following guidelines:

$0.01 - $99.99 – The account balance is due and payable within 30 days from the date of billing.
$100.00 - $199.99 – The account balance is due and payable within two (2) months.
$200.00 - $499.99 – The account balance is due and payable over a six (6) month period.
$500.00 - $999.00 – The account balance is due and payable over a nine (9) month period.
>$1000 – The account balance is due and payable over a twelve (12) month period.

Tallgrass Surgical Center expects unpaid balances to be addressed in a timely manner. The Business Office staff will develop a flexible payment agreement, should the need arise.
